You have the right to access and obtain copies of your health records. To request these, please follow these steps:
- Submit a Written Request: You must provide a written request to our office. You may do this in person, by mail, or via email at info@ccchiro-sa.com.
- Required Information: Please include your full name, date of birth, contact information, and specific records or dates of service you are requesting.
- Timeline: We will process your request and provide access as required by law, typically within 15 business days of receiving your written request.
